Accepted column names (header row of your CSV):

number or house_number  |  street or address  |  unit or apt  |  city  |  county  |  state (required)  |  zip or postal_code  |  latitude / lat  |  longitude / lng  |  full_address

full_address is auto-built from the other fields if your CSV does not include it.
